Nurse-Telephone Triage-Access Center PRN
Date Position Available:
Provides interactive clinical communication with callers during and after physician’s routine office hours. Offers personalized assistance with health management by providing information, education and guidance for patients that contact the nurse triage/contact center. Uses approved protocols. Acts as a conduit to secure electronic medical record system for patients that request appointments, prescriptions and consultation for routine and urgent needs. Has expert knowledge of KDMC services, including hours of operation and service provided. Practices exceptional telephonic skill. Uses expert interviewing skill, offers empathetic approach to caller. Coordinates patient office appointments, Urgent care visits, ER visits or admissions and offers expert support for clerk and staff. Has up to date knowledge of medication, procedures, diagnostics and treatments for a variety of diseases across the continuum. Includes Pediatric and Adult practice skill. Participates in data collection and interprets information to apply to improvement of processes. Collaborates with all departments and disciplines to meet the needs of the patient and family Demonstrates expertise in assessing social, psychosocial and physical /medical needs. Keeps ongoing communication with all providers, works with interdisciplinary team. Promotes KDMC by facilitating communication and development of rapport when working with customers, and various health care providers and institutions. Advises callers and directs them to the appropriate level of care by following accepted protocols. Follows protocol for best practice according to telephone triage software that will serve the patient. Charts in a timely manner in the EMR, providing clinical updates. Does follow up call within 24 to 48 hours of initial contact.
RN, 3-5 years of full time acute care nursing experience, BSN preferred but not required.Current RN licensure for State of Kentucky, Ohio and West Virginia. BLS required.
While performing the duties of this job the team member is frequently required to stand; walk; sit; use hands to finger, handle, or feel; reach with hands and arms: climb or balance; stoop, kneel, crouch, or crawl; talk or hear and taste or smell. The team member must regularly lift and or move up to 25 pounds, and occasionally lift and/or move up to 50 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, and depth perception.
